The Basics
- Social Security Card
- Birth Certificate
- State ID
- Green Card/Work Permit
Other Helpful Documentation
- Homeless certification from Shelter
- A letter from an institution that the individual has been released from
- Eviction notice
- Copy of foreclosure notice
- Copies of utility shut-off notices
- Copies of most recent pay stubs (last 4)
- Most recent federal or state tax return
- Copies of most recent dividends or interest statement
- Copies of most recent benefit notice, pension statement or other payment statement
- Copies of most recent unemployment or disability payment statement
- Copies of most recent TANF or public assistance benefit or payment statement
- Copies of most recent alimony, child support or foster care payment statement
- Copies of most recent Armed Forces payment stub or statement issued by the government

Please bring with you all/any that apply to you in order to receive assistance as quickly as possible. If the documentation requested above does not exist but is needed there are other methods of documentation that will be deemed acceptable.
